MDH, Health Promotion and Chronic Disease Strategic Planning and Engagement

The purpose of the strategic planning effort with this key division of the MN Department of Health was to develop goals, strategies, and actions that will protect, maintain, and improve the health of Minnesota’s population. A health equity lens helped ensure the division’s strategic plan and actions are designed to reduce health disparities.

Beginning in April 2022, we jointly planned the process, reviewed and assessed related plans, worked within the larger MDH strategic context, gathered perspectives and ideas from all staff and key partners, and conducted rigorous analyses including a comprehensive SWOT. That initial work was virtual per COVID restrictions. As people once again began convening in person, we used all that guidance in multiple workshops to collectively draft HPCD’s vision, mission, goals, and strategies. Throughout the process, kept stakeholders informed, sought feedback on drafts, and finalized the plan together by mid-2023 (HPCD Strategic Plan Final Report).

In 2023 and 2024, we are actively supporting collaborative implementation. This includes developing detailed, section-specific action plans and logic models to advance the strategies and goals; identifying division-wide collaborative opportunities aligned with the action items; developing key performance indicators along with monitoring protocols and visuals; conducting organizational analyses; and developing routine reporting processes and reports.
