A health care home is an approach in which primary care providers, families, and patients partner to improve health outcomes and quality of life for individuals with chronic or complex health conditions. The Health Care Homes (HCH) program within the MN Department of Health supports advanced care through clinic certification and level progression, support from program staff, learning opportunities, and patient resources.
As healthcare in Minnesota has evolved over time, the HCH program needed a strategic plan that reflected those changes and looked farther forward, positioned the health care home approach as a robust model, and clarified the program’s role working with primary care organizations and partners to advance and sustain equitable, patient-centered care.
With a commitment to equity and inclusion, we jointly planned the process within the larger MDH strategic context, gathered perspectives and ideas from all staff and key partners, and conducted rigorous analyses including a comprehensive SWOT. We used all that guidance in multiple workshops to collectively draft HPCD’s vision, mission, goals, and strategies. Throughout the process, we kept stakeholders informed, sought feedback on drafts, and used those results to refine guiding documents and support implementation.
